I understand that all reasonable safety precautions will be taken at all times during Kids Church and that the Kids Church volunteers cannot be held responsible for personal injury, loss or damage incurred by my child.

I understand that if urgent medical attention is needed, that every effort will be made to contact the child's emergency contact, as per this form. In the event that contact cannot be made, I authorise the leader in charge to take such action as may be necessary, including the calling of an ambulance and treatment by paramedics. I also authorise the Medical Response Plan as outlined above. I accept responsibility for all expenses associated with such treatment.

I understand that I am responsible for my children following the Sunday morning worship service (i.e. at all times outside of Kids Church) including if they should they participate in any informal, unorganised, and unsupervised activity in the Centre.

I also give permission for my child/children’s image, either on video or still photography to be used for church purposes.
