By signing this form, I understand that:

• all reasonable safety precautions will be taken at all times during the program and that the volunteers cannot be held responsible for personal injury, loss or damage incurred by my child.
• if urgent medical attention is needed, that every effort will be made to contact the child's emergency contact, as per this form. In the event that contact cannot be made, I authorise the leader in charge to take such action as may be necessary, including the calling of an ambulance and treatment by paramedics. I also authorise the Medical Response Plan as outlined above. I accept responsibility for all expenses associated with such treatment.

I also give permission for my child/children’s image, either on video or still photography to be used for church purposes.
